Objective of Position:
The Resident Services Coordinator will develop and implement programs that will serve seniors living in our apartment community. Housing with Heart is seeking a person to work 19 hours a week.
Major Areas of Accountability:
- Assess the needs of residents via a resident interest survey and personal interviews.
- Plan and direct on-site resident meetings, social activities, and educational programs for seniors.
- Identify community resources that will help the residents maintain their housing—such as low-cost health insurance, food programs, employment, government entitlements—and assist them with the applications.
- Contact and provide referrals to outside agencies.
- Develop a calendar and articles for the monthly community newsletter.
- Assist the residents in using computers.
- Recruit and supervise volunteers.

Level of Education, Experience and Skill Set:
- Excellent communication skills.
- Over 21 years of age.
- A.A. Degree; or pursuing a BA degree or equivalent; or equivalent experience serving seniors.

The ReConnect Program is designed to identify and reduce mental health symptoms and risks in isolated older and disabled adults in Orange County. It integrates evidence-based practices into culturally diversified care management and socialization services, which include mental health screening and assessments, client and caregiver education, referrals and linkages to community resources, and behavioral activation. The goal of the program is to increase individual's protective factors by reconnecting them with meaningful community activities. Consistent exposure to reinforced healthy life patterns has been proven to help sustain good mental health status.
Objective: To improve the personal and social functioning of isolated adults and older adults by providing comprehensive psychosocial assessments and developing customized socialization plans to decrease the symptoms of depression and other mental health illness.
Major Responsibilities:

• Good written and verbal communication skills.
