The goal of the program is to increase individual's protective factors by reconnecting them with meaningful community activities. Objective: To improve the personal and social functioning of isolated adults and older adults by providing comprehensive psychosocial assessments and developing customized socialization plans to decrease the symptoms of depression and other mental health illness.
